What Causes Irregular Menstruation?

i am 46 years old and my periods have always been more or less every 27 days until the now , i was a week late then it was only a little bleed once a day for about a week and then it has been a steady flow heavier than my normal period would be and there are a few small clots when i go to the toilet!!! any idea what is going on?
