What Causes Irregular Periods Along With Heavy Vaginal Bleeding With Clots?

I m 53 yrs old and have been having missed periods on and off since June 2014 my last menstral cycle was 12/2014 lasted 14 days My period started 03/24/15 and it started normal the first 7 days now it heaving bleeding with clots What can be causing this??
