What Causes Irregular Periods, Mid Cycle Bleeding With Ovary And Uterus Pain?

I need help, URGENT! I'm 21, no children. I started my period when I was 11, they have always been very painful and heavy. My mother and grandmother suffered from the same and both had to get oblations. My mother had cysts (uterine or ovarian) when she was about my age. I just started my period again in December after not having one for over a year. I am experiencing bleeding in between my periods, at the moment it happens every couple of days. I am having pain in both my ovaries and my uterus and today when I though my tampon was full and went to go change it, it wasn't full of blood it was a clear watery fluid. FULL of that fluid, I felt it gush and overflow when I coughed. Also when I cough I feel a "POP" in my uterus, like popcorn popping and once it was accompanied with bleeding after I felt the "pop". What could this be?
