What Causes Irregular Spotting, Abdominal Pain And Frequent Urination?

50 years old, never missed a period and no typical signs of menopause yet. Endometrial thickness measured to be 1cm with mid-cycle bleeding and periodic severe abdominal/back pain often waking me up at night. Also blood work shows excessive vitamin b 12 at 1300 and low TSH at 0.25. Other symptoms include frequent urination, headache, fatigue and diarrhea. what could this mean and what follow up should be next?
