From history it seems that she might be having yeast infection which is common due to poor local hygiene at this stage of pregnancy as there is more bulk of thighs and fullness in lower abdomen.
Clean local part with warm water and apply anti-fungal cream.
Keep local part clean, dry and airy.
Avoid more perspiration.
Ok and take care.
