What Causes Light Spotting Followed By Heavy Bleeding?

i have irregular periods. normally my first 2 days are extremely painful but it gets better. im on my period now and they were not painful at all. my first couple days were light, but now im on my 5th day and i cant go 40 mins with out changing my tampon and bleeding everywhere..what could it be?
