General & Family Physician 's  Response
Hi, thanks for using healthcare magic

The cold sensation may be related to his infection. Infections can cause the sensation of chills.
His low blood pressure would also play a role in feeling cold.

It would be best for him to go either back to his doctor or go to the hospital.

He may need fluids to be given to help raise his blood pressure along with anti pyretic medication (fever medication) and possibly iv antibiotics.

He should recover with appropriate treatment.

I hope this helps
