What Causes Lower Abdominal Pain Incomplete Urination?

My 15 year old daughter has occasional severe lower abdominal pain. She will feel the need to urinate and then won t be able to. She ends up with severe pain, curled up in a ball crying. This happens to her about once a month or so. We have a urologist who has conducted every test there is (invasive and otherwise) and everything comes back as normal. Any suggestions on what our next step should be. We ve been with the urologist for about a year. She s seen a gynecologist and all tests were negative. She also saw a doctor to see if she had diabetes, again negative.

Hi,

Possibilities are
1. Imperforated hymen or block to the outflow of blood, causing severe pain. This is if she has no periods or minimal flow or only discharge.
2. The early appearance of endometriosis. possibilities are low. But the pain is close to periods.
3. Hope a scan of abdomen and pelvis has been done. MRI would be more informative, as the scan may miss minute findings.
4. Hope urine culture and sensitivity is done.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. R.Balakrishnan, OBGYN
