Upper
respiratory tract infection (URTI/URI) is most commonly due to virus spanning multiple virus families and many antigenic types.
Rhinovirus is the most common cause of URI (~30–40% of cases); other causes include
influenza virus as well as para-influenza virus, coronavirus and adenovirus.
Respiratory syncytial virus (RSV) is also a recognized cause of significant disease in elderly. A host of additional viruses, including some viruses not typically associated with URIs (e.g., enteroviruses, rubella virus, and varicella-
zoster virus) account for a small percentage of cases in adults each year.
Nonspecific URI commonly presents as an acute, mild, and selflimited catarrhal syndrome with a median duration of ~1 week (range, 2–10 days). In the absence of clinical evidence of bacterial infection, treatment remains entirely symptom based, with use of decongestants and non-steroidal anti-inflammatory drugs (NSAIDs). In a significant minority (particularly the elderly), however, symptoms of weakness or lassitude (postviral asthenia) may persist for several weeks.
