What Causes Lower Back Pain, Insomnia, Chest Pain And Swollen Testicle?

I have very tender testicles, some testicular swelling, lower back pain, very bad breath, some chest pains, insomnia. What should i be looking for? I m a white male 53 years old, 6 2 , 195 lbs I stopped taking zoloft about 3 weeks ago, and used to take hydrochlorothiazide until my blood pressure went down(most likely because i dropped about 45 pounds last summer)
