Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.
Hitting hardly on the back of the horse might have led to local injury and swelling on vagina. You may need to take analgesic drugs to decrease the pain, antifibrinolytic drugs to decrease and prevent the bleeding, antibiotics to prevent the infection. Sometimes rarely blood clots in other parts of the body can travel to heart which is called as embolism, but this may not happen every time.
But better to be careful, get examined by your doctor to find out the exact cause of the problem and take treatment accordingly.

Take care.
