What Causes Migraines Associated With Dizziness And Blurred Vision?

My friend is 63 yrs. old. He s about 6/5 . Wgt: 210. He sometimes has very painful migranes accompanied by light sensitivity & diziness & blurred vision. He frequently has vertigo like spells that last for a day -3 days before they are completely cleared up. He feels extreemely dizzy & can t stand up much during those times. Those spells are accompanied by black & white dotted vision & blurry vision & moderate - severe weakness in the body. He s not a diabetic that we know of or anemic. He eats normal/well. He is a workahaulic & always traveling & working long days non-stop almost. He hardly ever sleeps. His mind is always working. What could this be? He has an issue I no about but not sure what it s called. He was diagnosed with it as a child. One of his valves in his heart doesnt close or otherwise follow natural function. He has no heart surguries or other aside from sinus surgury that failed & now he needs another one. they tore his fine membrane in his sinus cavity so it never healed.. he lives in Asia.. nice polluted place to have that sinus cavity open.. I think i remember him saying that his heart is larger than normal as a side effect of whatever the valve issue is. He has no health insurance so what could this be & where can he get an EEG.. ECG & blood work done to check his liver, kidneys & anything else for free?
