What Causes Mild Pain Along With Yellow Fluid Drainage From The Ears?

Hello - Nearly 6 months ago, I woke up one morning with the right side of my face, concentrated around the ear, feeling numb or full. My hearing from the ear was a bit muffled. A few days later, I woke up in the middle of the night in pain with a very light yellow fluid draining from my ear. I tilted my head over a pot of steam and drained a large amount of fluid, which relieved some pressure, then went to a doctor in the morning. Low-frequency sounds were about all I could hear and they were aggravatingly loud, my friends told me I was talking too quietly for them to hear (which is not how I d expect someone to compensate for hearing loss). I live in Thailand and struggle to communicate effectively with the more affordable doctors who speak little english. But the doctor told me it was an ear infection that built up, gave me antibiotics and ofloxacin eardrops. I saw another doctor a week later who agreed and gave me more eardrops. So far so good, my hearing slowly returned over a few months but the pressure in my face remained. It felt like the air pressure around my ear was different than the air pressure in the rest of my head, and sometimes, especially at night, it felt like the pain/pressure was moving around my head (perhaps as I subconsciously flexed muscles in my face). It would move from my ear to my upper jaw and down to my mouth (eustachean tube?) and was not terribly painful. The pressure also got better but was still there, my hearing returned to 90%, though there are days when it feels like one or the other of my ears just opens up, and when that happens, I m always amazed by how much more I m hearing. Fast-forward and nearly six months later, I went to see another doctor to check on the ear and ask about the pressure. He said my eardrum had healed and that perhaps I ve had allergies without knowing it, and gave me Zyrtec and Pseudoepenepherine and a nasal spray. The week I took these, I felt great. I could feel things moving around inside of my head, sometimes painful, exerting some force on my eyes and both ears, but I thought this was at least a sign of progress, maybe whatever was clogging my face was going to come out? I was able to cough up some white phlegm, and also had a pretty constant postnasal drip though I can not seem to get anything to come from my nose. A week later, the pressure and pain were worse. My jaw feels like it is not in the right place sometimes. The pressure behind my eye is very painful and I find while reading that I m mostly relying on my right eye, though the left one still seems to work when I close the right one. Last week I went back, got a different doctor who looked up both nostrils and my right ear with cameras on tubes, and said my right ear was definitely still ruptured and that it and my nose contained a lot of white mucus/fluid. I don t know why the doctor didn t see this a week earlier, but during the week I had resumed getting my ear wet during showers, listening to headphones, etc. having been assured that it was OK. The new doctor gave me more Zyrtec and another antibiotic. Now it is one week later and my head never feels normal. The pressure is acute, my eyes and ears hurt, my cheeks and jaw hurt, and the top of my neck and bottom of the jaw, under the tongue, is swollen. The area above my teeth and under the nose is also very painful. I am having some difficulty swallowing, still nothing coming out of my nose, still coughing up white phlegm and postnasal dripping white fluid, and still feeling not myself. I have cut the nasal spray and am using saline water instead. I m nearly finished with this course of antibiotics and allergy medicine, but I honestly don t know if they are helping or making it worse. Are the feelings of something moving around under my face and pressing against it side-effects of the medication, or a sign that the medication is working? Is this really just an allergy (though I ve never noticed I ve had allergies before, I have always been pretty decongested and when I have those rare days where everything feels OK, I feel like I am smelling and tasting better than I have in my whole life). I am completely confused, don t know if I should continue antibiotics or allergy medicine or find a different doctor, but I feel like I m going crazy and am not able to articulate myself well to the doctors here. What should I do? Possibly related, two weeks ago I went to a dentist for the first time in maybe three years and she removed a cavity that has possibly been there for quite some time. Thanks so much for reading and offering any kind of recommendation!

Hi,

I am sorry to hear about your long suffering. From what you have mentioned, it appears you have an underlying Rhinitis or Atopy which means you have nasal congestion most of the time.

This had put you to risk of developing Otitis Media (Infection and inflamation of middle ear cavity) which has been a recurring problem for you and now this condition we call as CSOM (Chronic Suppurative Otitis Media).

Treatment modalities vary depending on multiple factors. Sometimes medicines alone may not be enough. What you need is a complete evaluation by a Head & Neck Surgeon along with Imaging mostly CT SCAN after which treatment plan can be made.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
