What Causes Muscles Aching, Fatigue With Legs Swelling?

I wake with achy muscles and extremely fatigued (which persists), my legs are swollen by the afternoon most days of the week and I've developed exercise intolerance. I've also been to several doctors and many conditions have already be ruled out, but they've also given up on me. Ideas?
