General & Family Physician 's  Response
Hi.

Probably diabetic neuropathy requiring some attention to his blood glucose profile. If his HbA1c is found to be abnormal, then a look into his blood glucose profile along with better correction of his blood glucose could prevent further progression and worsening of his condition. An elaborate cardiac profile should also be conducted simultaneously to make sure his heart is not contributing in any way to his presentation.

Best wishes.
