Hi,I am Dr. Shanthi.E (General & Family Physician). I will be looking into your question and guiding you through the process. Please write your question below.
What Causes Oral Ulcers And Lumps In Esophagus Of An Elderly?
My 84 year old mother in law, with dementia, is currently hospitalized with acute ulcers covering her gums and tongue. They have also found, through and endoscopy, lumps in her esophagus and infection. They have sent that out for testing. She has been taking sulfasalazine for years and they are thinking possibly she has developed a severe reaction. Prior to being hospitalized the diagnosis was coxsackie virus then it was changed to thrush. The also complains of a dry mouth, sensitivity to spices, and her food has no flavor (not true) so she is dumping honey on everything.
Based on the history you have shared, she was on immunosuppressive therapy which can trigger fungal infections like oral thrush or candidiasis which can extend downward to cause esophagitis, etc.
She may need to be started on oral antifungals like fluconazole as well as Nystatin drops for gargles and swallowing.
Meanwhile, antacids like Omerperazole as well as motility agents like Domperidone or Itapride before meals may also be required along.
Hope I have answered your query. Let me know if I can assist you further.
Take care
Regards,
Dr. Saddiq Ulabidin, General & Family Physician
