Welcome at HCM...
I have gone through your query and being your doctor I completely understand your health concerns....
For how long you have been diagnosed with crohn disease.??? Which medications you are using??any comorbidities???
Crohn disease has got remissions and exacerbations.. Based upon the history you have provided I don't think you are developing any new fistula although crohn disease is notorious for fistula formation... Have you met with your surgeon?? Since u already had fistula and that may be contributory factor in giving you urinary tract infection which is causing you pain....
Consult your surgeon for proper management. .
Hope your query is adequately answered...
Regards and stay safe
