What Causes Pain And Tightness In The Chest?

I am experiencing pain and tightness in my chest and somewhat in my back. There is pain under my arms running through my pectoral muscles, I have a slight headache, and my arms feel heavy and fingers are occasionally tingly. I am hungry but when I eat I feel nausea and/or actually throw up. This has been going on for about 30 hours. I do suffer from GERD and take Nexium and antacids.
