What Causes Pain Around The Anus And Burning Urination?

I have pains which seem to come from urethra but cause deep aching around anus and nerve spasms. My urine seems to burn outer vaginal area. I am 68 yrs old. I have had 5 doctors including a Gynocoligist check for vaginal problems and the only answer I have been given is vaginal atrophy. However this problem is much more painful and includes more than one type of pain; from burning to deep aching to nerve spasms. Estrogen creams, hemorrhoid treatments, have no results. Rinsing after urination and applying Coconut oil seems to help as a barrier but does not prevent the deep aching. Heat seems to help the aching but doesn t clear up the redness or burning. Today I did a deeper self exam and noticed an elongated growth in the deep fold at side of the urethra. This is the 3rd bout I have had of this painful condition. The condition lasts for months, both clearing up and starting again with no apparent reason.
