What Causes Pain During Sexual Intercourse?

Hi.I gave birth to my son nearly 7 years ago and had to have an episotomy.I had a cervical smear 3 years which I found extremely painful.I went for my smear today which was unbearable.I normally have a good pain threshold but this was unbearable what I experienced today.The nurse was unable to do the smear and advised that I book with a female gp.I also have pain on sexual intercourse and as a result this has affected my relationship with my husband.What advice can you give me and is this something that can be sorted?.I am 40.Height 5 foot 8,weight 11 stone.I have hypothyroidism.

Hello,

Thanks for writing to us.

According to your information, painful intercourse may be due to following reasons:

1) Endometriosis, 2) Pelvic Inflammatory Disease, 3) Dryness of vagina 4) blunt trauma or rough sex, 5) Vulvodynia 6) Vaginitis etc.

You need to undergo proper clinical or pelvic examination and necessary investigation after consultation with your gynecologist. It helps to confirm the diagnosis.

Initial investigations include pelvic ultrasound scan, urine routine & culture test, vaginal swab test, hormone profile etc.

TREATMENT: Proper treatment can be started following diagnosis of the case. In the mean time, I suggest to take NSAIDs or painkiller if pain is quite non-tolerable.

Maintain genital hygiene properly, avoid mental stress as much as possible by regular meditation, do unprotected sex with proper lubrication (use K-Y jelly), avoid chemical bath products, wear clean & cotton underwear, drink plenty of water to flush out body toxin.

Hallow Dear,

If painful intercourse is dating back to your last delivery during which you had episiotomy, we can blame unhealthy healing of episiotomy leaving a painful tender scar. This scar is causing you pain whenever something - may be penis or may be spatula - is trying to entre the vagina. Due to tender scar you have developed vaginismus.

I feel you should report this to your Gynaecologist. If the scar is tender even after 7 years, you need some attention to it. If it is chronically infected, some antibiotics with anti-inflammatory medicines will be required. Sometimes local injection of hyluronidase is useful for tender fibrotic scar. Injections of placental extraction are also claimed to be useful for such bothersome fibrosis and inflammation.

I hope this gives you some guidelines for management.
