What Causes Pain From The Chest To The Armpit And Back?

my husband has been have chest pain that radiates under his right armpit, across his back and up into his neck. He has had the following treatments that have not resolved the pain. 1. removal of his gallbladder 2. Just saw a cardiologist that did an echo, CT scan and could not tell of anything. what should be our next step - he cannot continue with this pain.
