What Causes Pain In Back Of The Testicle While Urinating?

i am a 20 year old male with visceral pain towards the back of my testicle, not sensitive to palpation, no abnormalities upon physical exam, and no fever. i did have a knee surgery in the past and the pain increases with walking/running. and i am having pain with urination. im not sexually active but i do masterbate

Thanks for your query.
Noted your history of pain at the back of the testis on urination but there is no local tenderness in the scrotum.
You have painful urination. This indicates that you are most probably suffering from urinary tract infection and may be a stone somewhere.
I would advise you the following:
Send the sample of urine for culture and sensitivity and get started on antibiotic and symptomatic treatment.
Ultrasonography of the abdomen and the scrotum.
Further investigations and treatment as per the findings under the care of an Urologist.
