Hi there... i am dr.saddiq..i would like to look into your matter in detail.
Brief answer:
It is a multiple speciality domain and these symptoms shouldnt be taken lightly.
Detailed answer:
Putting on a cardiac monitor and visiting many doctors does infact indicate that you must have been through alot, but i would like you to spare some time and re-think all the details of this recent work up and then re-evaluate your situation based on few of my suggestions:
1- Your symptoms like palpitations, chest pain leading to passing out does reflect likelihood of a cardiac syncopy for which along with holter monitoring a stress ECHO and ETT would rule out any cardiac ishemia secondry to coronary artery vasospasm or
infarction (which may be contributing towards inability of your cardiac muscles to suffice its oxygen demand and leading to its exhaustion) and a detailed
lipid profile. If these are not suggestive then after visiting and discussing with a cardiologist you can evaluate the plan for a
cardiac catheterization.
2- The second possibility can be that your nervous system is contributing towards symptoms like passing out though that needs further evaluation with a detailed history about preceding and proceeding events before such incidence. Any history of
incontinence,
tongue bite or fits? A detailed history by a person who is witness to such incidence? Any auras or headaches before such incidence? Any positive family history? and also a series of tests like your electrolyte levels, Thyroid profile, Prolactin levels, EEG and CT scans or MRI if all others are not conclusive.
3- Another likelihood might be that your brain doesnot get enough blood supply after such event and it leads to passing out so all the major vessels supplying your brain needs to be evaluated via carotid doppler imaging as well as aortogram or CT angio to rule out coaractation of aorta (as chest pain with syncopy rises its possibility) or carotid plaques. Also before such extensive investigations, a detailed Blood pressure evaluation and comparison from all four limbs, along with a detailed cardio vascular examination and detailed auscultation of heart to rule out any
valvular heart disease.
I have tried to summarize maximum, possibilities to your query, but you need to visit a cardiologist and a
neurologist for further discussion regarding all these possibilities so that they can examine you in detail keeping in mind all these possibilities and your relevant queries.
