What Causes Pain In Neck And Shoulder?

I have some neck-shoulder (right) linked pain while i do some careful work like stitching, chopping, etc. What could be the reason? It pains so badly that my neck gets frozen n it becomes normal (sort of) when massaged. I used to have a swelling on that shoulder since 16. and I tried many medicines (ayurvedic, homeopathy) - reduced swelling - less noticeable but the small swelling left bothers me, which is where I feel pain.
