What Causes Pain In Penis Head When Pulling Foreskin Back?

I get a sharp pain on the top of my penis right below the head, when I pull back my foreskin. The pain has gotten worse since last night and make it really uncomfortable to urinate. Should go to the er and what could it be. My ferelum is already ripped, so that is not what is causing the pain. I am married, so I highly doubt it is a STD.

The pain could be due to inflammation or accumulated smegma. It should be cleaned and diagnosed for pain. Therefore, I suggest consulting a urologist for physical examination, diagnosis and treatment.
