What Causes Pain In Shoulder/back/chest/waist/neck?

i am 40 years old,5-6 in height,138lbs.iam suffering from shoulder pain,back pain,waist pain.iam having chest pains sometimes and feels always dizzy specially in the morning.ialso have neck pain.i had this injury from basketball 2 years ago.iam suffering from these pains for 1 year now,i already had laboratory tests,ecg,x rays,andand its all clear.Where does this pain come from?
