What Causes Pain In Shoulder Blade After An AC Separation?

i had an ac separation 6 months ago and now have developed pain in the shoulder blade area as it feels like its 'grinding' when i move that joint, the ac joint feels as bad as ever day to day activities like opening doors and carrying even light shopping bags are painful and i have a constant aching in the area too.
