Hi Saju,

Welcome to HCM.

Normal level of blood triglyceride is less than 150mg/dL. Based on the information you have provided your blood triglyceride levels of 320 mg/dL and are certainly high due to the reasons such as intake of fried food. Long term elevated levels of triglyceride will impair the ability of a given artery to dilate upon increasing the energy demand. In your case are the coronary arteries. Thus, when you increased the demand on the heart by running and/or climbing a staircase, your coronary arteries could not potentially keep up with the demand with the resultant chest pain. In other words, you are at a higher risk to develop heart attack in future.

In similar cases, I have advised fenofibrate and statins (simvastatin or atorvastatin) to reduce elevated triglyceride levels. It is very important that you reduce your intake of fried food and increase intake of vegetables and fruits, and foods rich in fiber. I cannot stress less that you should at least do brisk walking every day for 30minutes. Just to rule out whether you have any impending atherosclerotic plaque building in your coronary arteries, I would recommend getting an angiography done as well as an exercise stress test. Your further nature of management may depend upon the results from these tests.

Hope this helps.
