What Causes Pain In The Lower Back Followed By Fever And Night Sweats?

I started having lower back pain on my right side then it went to a fever, night sweats, breathing problems like on my right side, now it hurts on both, I have had a temp of 100-102.7 the 102.7 was just a little bit ago my spleen is enlarged(I do have sorcidiosios) have had it for 3 years I m my spleen, I also have 2 spleens. I also started bleeding vaginally which makes no sense when I already had my period. I have sever pain my head hurts so bad oh I m very lightheaded, nusea, neck back leg and head on right side hurt.
