What Causes Pain In Upper Left Hand While Sneezing?

hello I am 68, female and last November I came off my bike whilst riding through the woods. I had severe pain in upper left hand back and front. I attended a A+E hospital who prescribed pain killers and discharged me. I contacted my own gp who prescribed more pain killers in December. I again contacted my gp again in March still suffering with pain, especially when I sneeze, or yawn. He checked my breathing and suggested an injection in the shoulder. How can I get a second opinion as I am still in considerable pain. I rejected the offer of an injection.
