Hello and welcome to HCM,
Pain in the lower back which increases while sitting and coughing is suggestive of musculoskeletal problem.
However, a X-ray as well as MRI of spine especially lower back is required to assess the condition of bones and soft tissues.
The pain can arise from musculoskeletal problem or problem of spinal nerves or intervertebral discs.
Thus, clinical assessment and relevant investigations are required to diagnose the condition.
Consult your orthopedician for further management.

Thanks and take care
