What Causes Pain On The Top Of Shoulder?

The top of my shoulder has hurt for over a month now. It's gThe top of my shoulder has hurt for over a month now. It's gotten worse and it's only one shoulder. It's the bone that seems to protrude at the intesection new the AC joint. If I press on the bone it hurts or if I move my arm any way it hurts.
