What Causes Pain Under Shoulder Blade And Armpit?

I am currently experiencing moderate to strong pain under my left shoulder blade and inside my left armpit. This has been ongoing for the past four days without reprieve. At what point should I seek medical attention? (Fyi: I quit smoking on Dec. 30th 2010, and was a pack to a pack and a half smoker for 8 years.)
