What Causes Pain While Taking The Tampon Out?

I used a tampon for swimming for a short while when I took it out it was almost dry. For the next 3 weeks I had pain. Then today as being in my period I used another to go swimming for a short while and again it came out almost dry and it was very painful to take out. Now the pain has been pretty bad for the rest of the day. Should I be seeing my gyno because of possibly tearing my vaginal wall..
