What Causes Painful And Swollen Underarm?

i am 17 and i have had underarm pain for a year and half now. some days are worse then others and i have been paying close attention to it in the last 6 weeks and everyday except once i had pain. swelling has occurred and the pain has gotten so bad that i couldnt put my arm down for a day or two. what does this mean and should i be concerned
