What Causes Painful Erection And Peeling Penis Tip?

Hello doctors ! My question is dis..when I was growing up I live in a neighbourhood were 5-10 share a bathroom and toilet which is very unhealthy as I now know. But I contacted an infection between my penis and my laps which at night I have to scratch till I injure myself at times but as time goes on I bought an antiseptic and medicated soap which I use in bathing and when I am thru I apply it on my penis and my laps allowing it to dry off to help me cure d infection. But after some years I found out that the cap of my penis is always peeling off ..something like dry skin coming off it and sometimes when I have an erection when having sex it hurts me and I found dat I have cuts arround the cap line round my penis and it hurts me bad..what help or reconmendation can u give.thanks
