What Causes Painful Erections?

my fiance has been out of the country for months. He experienced pain with erections while sleeping for about 5 nights in a row. sensation like a pulled muscle . However, while masturbating to make sure there wasn t an issue with his functionality, there was no pain. When returning home, he showed me and his penis is now bent and the lower shaft is hard and smooth while the upper half is ribbed, not smooth. Sorry, hard to describe. No pain any more but there is a definite change to his penis.
