Anesthesiologist 's  Response
Hello,

The following are few causes of severe pain during menstruation and heavy bleeding:
1) Uterine fibroids
2) Endometriosis
3) Adenomyosis
4) Uterine polyps
5) Pelvic inflammatory disease
6) Ectopic pregnancy
7) Ovarian dysfunction leading to anovulation

As you know you have fibroids, the following are treatment options:
1) Removal of fibroids
2) Hysterectomy if family is complete
3) Pain medication like tab. Mefthal spas daily twice during menstruation
4) Tab. Tranexamic acid for 5 days during menstruation.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Pallavi M., Anesthesiologist
