What Causes Persistent Chronic Pain In The Shoulder?

I have severe pain in my shoulder almost constant. It hurts when I breathe or even pass wind. I have been having pressure like an uncomfortable feeling in the centre of my chest for a couple of wks and that has flared at times. The shoulder is very painful. Sleeping is so uncomfortable. I woke with stabbing pains in my back and under ribs also. I thought I had strained my shoulder and have been waiting for it to ease.
