What Causes Persistent Dizziness Along With Fatigue And Headaches?

About 3 weeks ago I had a bad headache that was followed by a slight panic attack. . Ever since then I have had a roller coaster ride when it comes to feeling good. I start off the day feeling good and by the end of the afternoon I am exhausted and not feeling like myself. I have dizziness, fatigue, anxiety and headaches. My energy level is extremely low and I slept a lot. My head is a dull pain and the dizziness happens randomly.
