What Causes Persistent Dizziness?

Hello I m hoping you ll be able to help my girlfriend and shed some light on a situation.My girlfriend is Imogen Swift, she s 21. For the last month she s had feeling of dizziness which we haven t been able to get to the bottom of and its starting to really depress her and she s really struggling at the moment because she feels she can t cope with day to day life even though the symptoms are quite minor.Occasionally throughout the day she will experience short dizzy spells which last only a matter of seconds with certain head movements. So far it appears to occur with any random head movement, there doesn t seem to be a specific head movement which will trigger it. She s otherwise fit and health and on no medication. She describes the dizzy feeling as feeling as if she s on a boat and a little spin. We ve seen several GPs at her GP practice who just say it s vertigo. They ran her on a full 7 day course of antibiotics (Amoxicillin) thinking it might be an inner ear infection, this had no effect. They ve referred us to an ENT specialist suspecting BPPV, we attended the appointment and they said all is fine and she doesn t have BPPV and she passed the tests which they did which just involved laying her down quickly on a bed and turning her head whilst watching her eye movements.She s getting really depressed that it s not been diagnosed so far and she s still experiencing these momentary dizzy spells throughout the day, so much so that the hospital agreed to give her a CT scan of the head for her own peace of mind, the CT scan came back all clear with no abnormalities detected .She s got no trouble with hearing and she doesn t seem to have any trouble with her balancing either. Her eye sight is good, she has glasses but only for driving, her last eye test was just under a year ago.Over a year ago she did have a period where she experienced vertigo for about a week and had time off work, but this corrected itself after a week and she s been fine since, until a month ago.During this time she has had a baby, and this series of dizzy spells started approximately a month after having the baby. She never experienced any dizzy spells whilst pregnant, so we wondered if a hormone imbalance could cause them? For this reason the doctor has prescribed her the pill (Ovranett) with the hope this might help her hormones become balanced.Just recently however, within the last two weeks, she s started to have occasional panic attacks every couple of days, and she possibly has anxiety - but the only thing she s anxious about is the worry of what could be causing these dizzy spells. So far we ve seen countless doctors, an ENT specialist, and she s had a CT scan but so far nothing seems to have shed a light on what might be happening.She s tried various prescription tablets which help with nausea and vertigo but none of these have made any difference.I m hoping you might be able to shed some light on it.Many Many Thanks in advance,James

Hi,

Since the symptoms are persisting despite taking GP consultation and ENT examination, thus following is my opinion:
1. Taken an opinion of a cardiologist [for cardiovascular status], a hematologist [for blood picture], and a neurologist [for any neurological sign] for an underlying cause of dizziness
2. Cervical spondylitis is the commonest cause of giddiness (ask for X-ray cervical spine, ENT and ophthalmic checkup. (Additionally, go for thyroid profile), till then general advice for her will be:

** Avoid movements of the head, bed rest
** Avoid caffeine.
** Check whether any drug that the patient is taking (if any) is causing giddiness
** Use a cervical collar
** Injection neurobion IM [alternate days]
** Capsule B complex twice daily for 15 days

Check with her Vitamin B12 level since a deficiency in this age does cause neurological problems.
