What Causes Persistent Ear And Upper Respiratory Tract Infection In A Child?

My 4 year old twins have had double ear infection, upper respiratory infection, and sinus infection and a temp running about 103 for awhile now. They have been on steroids,antibiotics, breathing treatments and got a steroid shot. I m not sure why this is lasting so long. Is there a underlying problem ? The doctors here don t know what to do.
