General & Family Physician 's  Response
Hi,

Firstly, I want to know the age, sex and any significant personal history of hypertension, type 2 diabetes mellitus, hypothyroidism, sleep disturbances and any eye complaints with any significant family history of similar complaints. Also, the duration of entire symptoms will help in closing on the diagnosis.

Now, coming to the major complaints of yours. Considering all the complaints related to same etiology I can suggest three possibilities:
1. Migraine
2. Cervical spondylosis
3. Irritable bowel syndrome

In all three cases, I would definitely rule out tuberculosis involving both abdomen and central nervous system. We can proceed with the following investigations:
1.CBC/ESR/LFT/KFT/RBS/Chest X-ray PA view
2. Brain imaging with imaging of cervical spine as well (NCCT/MRI depending on affordability)
3. USG Whole abdomen
4. Thyroid profile (FT3/FT4/TSH)
5. Stool for Ova/cyst/routine
6. Refraction testing

For treatment we may start the following:
1. Tab Normaxin
2. Tab Flexon MR
3. Cap Pantop DSR before breakfast
4. Tab Bandyplus single dose
All medications for five days except 4
Hope to get well soon. We may review with investigations.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Santosh K. Pandey, General & Family Physician
