Hi..
Welcome to HEALTHCARE MAGIC..

I have gone through your query and can understand your concerns..

As per your complain it seems that there is SECONDARY CARIES in the filled tooth which must have extended till the pulp which is a central portion of the tooth and contains nerves and blood vessels and inflammation of the nerve causes severe pain which can also radiate to all the areas you explained in your question..
If it is the case get another x-ray done of the offending tooth so that the cause can be ruled out and if it is confirmed removal of the filling along with treatment with root canal treatment followed by antibiotics[ Augmentin or amoxicillin] and painkillers like Ibuprofen has to be taken..

If it is not the cause then the Right Maxillary sinus needs evaluated for infections that can lead to pain in teeth, gums ,jaw and an radiate to ear, eye, head and nose..
Treatment will be done by OTOLARYNGOLOGIST or ENT SURGEON and after a final diagnosis is made..

Even if sinus issue is also not there then looking for nerve problem like TRIGEMINAL NEURALGIA has to be ruled out..

You need consultation with an Oral Physician and get a proper evaluation and diagnosis done and get treated accordingly..

As of now you can take TRAMADOL [ULTRAM] 50 mg four times a day for pain control..

Hope this information helps..

Thanks and regards..
