What Causes Persistent Pleuritic Chest Pain?

I m a 38 year old male. I have had chronic asthma for my entire life as well as gerd. I am currently being treated for those successfully. I have had pleuritic chest pain that radiates to the right shoulder and neck. BP gets high as well as pulse after eating sometimes. The pleuritic pain doesnt go away, but is less sometimes. This has been going on for about a year and a half. My GP has sent me for multiple tests for my gallbladder, but that comes up normal along with blood work. I have had pleuracy in the past, but it was treated with Z pack. I have been on Antibiotics a few times in the past year or so after er visits due to diverticulitis. A CT earlier this year of my stomach did say Parenchymal scarring and Atelectasis. I have had two ultra sounds and a HIDA scan that were negative. This pain is on the right side and it goes to the back as well on the lower side of the lungs. Doctors cant seem to find an answer. I have had chest xrays that were normal, but no actual CT of the chest. I take fiber 2x a day, symbacort, albuterol and protonix. Thanks for any help

Hi,

Noted your history of well controlled asthma, GERD, known case of pleurisy with pain, pain in the chest radiating to shoulder and neck, treatment with Z-pack and other antibiotics in past, normal ultrasound of abdomen and HIDA scan, no actual CT scan of chest done till now. All your symptoms indicate that there has to be a reason for pain in chest and shoulder and neck. Hence I would advise you the following in such a situation:
Request your Doctor for CT scan of chest as there is history of Asthma, chest pain and pleurisy. This would show for any other parenchymal disease of chest structures that is lungs, pleura, heart, lymph nodes and so on.
MRI of the cervical spine will help to know the cause of neck and shoulder pain is from Cervical spondilitis.
Pulmonary function tests.
Tests of blood.

All these and any other additional investigations or clinical findings will help to get an exact cause and give a proper plan of management.
