General Surgeon 's  Response
Hi,

Based on your query, my opinion is as follows.
1. Due to shaking secondary to chills, sweating can occur secondary to adrenergic stimulation.
2. This excess stimulation along with dehydration could cause severe fatigue.
3. Good hydration is essential. Nutrition needs to be improved with multiple small meals.
4. Hypothyroidism is one of the common cause of excessive cold and fatigue.

He will require thyroxine and dosage needs to be increased slowly over a month. More clinical details and laboratory reports necessary for further opinion.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Ivan R. Rommstein, General Surgeon
