What Causes Poor Eyesight, Impaired Hearing And Impaired Intelligence Since Birth?

What can cause a child to have all of these symptoms from the time they were born poor eyesight, deep set eyes, significant hearing impairment, stunted growth, heavy facial features, intellectual impairment (presenting with learning problems, mental immaturity, poor intellect) and speech issues?

This could be Cerebral palsy and Mental retardation. He needs treatment by Paediatrician, Physiotherapist, Psychiatrist.
