What Causes Poor Weight Gain In Infants?

My baby was born a late preterm and was 2.6 kg at birth. She is now 13 months and weighs just about 7.9kg. She crys before dating and not walking independently yet. My question is what can I give her to help her weight and can I also deworm her now. Thanks, Janet.
