What Causes Posterior Lateral Knee Pain With Deep Flexion?

I ve been having posterior lateral knee pain with deep flexion, weight bearing hyperextension and with firm pressure on the area for the last 3 months. No known injury. No DVT on US. MRI of knee negative for medial and lateral meniscal tear. 4mm near full thickness cartilage defect seen on medial femoral condyle. No significant effusion. Pain is inconsistent with cartilage defect. What could this be??
