I think everything is getting mixed up here because of one simple mistake of not correcting the hip flexor muscles. These muscles tend to become shorter after the L4-L5 disc degenerative changes.
Over a period of 20 years, such a long time, this muscle would have gone into tightness and might be applying pressure over the vessels passing underneath.
It's common that most people after the
disc injury have shortening of the hip flexor muscles and the other symptoms gets add on.
I will advise you to stretch the hip flexor muscles and then see how you feel altogether. In fact, in my clinical practice of over 12 years, I have seen cases having secondary issues post disc injury.
The diagnostic tests like
MRI,
ultrasound scanning, blood, and urine test are routinely done to understand the pathologies and once finding this hip flexor tightness as the cause and working behind it and lower
limbs as a whole we get good results of symptoms reduction.
